在肥胖與代謝醫學領域中,脂肪的分佈位置往往比脂肪的總量更加關鍵。人類的脂肪主要分為皮下脂肪(Subcutaneous Adipose Tissue, SAT)與內臟脂肪(Visceral Adipose Tissue, VAT)。皮下脂肪位於皮膚下方,主要功能為儲存能量與禦寒;而內臟脂肪則主要沉積於腹腔深處,緊緊包覆著肝臟、胰臟、胃及腸道等核心器官。 從組織學與內分泌學的角度來看,內臟脂肪並非只是靜態的能量儲存庫,而是一個高度活躍的內分泌器官(Active Endocrine Organ)。當內臟脂肪過度蓄積時,會異常分泌大量的發炎因子與游離脂肪酸,直接經由門脈系統(Portal System)流入肝臟,引發全身性的代謝連鎖危機。 內臟脂肪過量的評估標準與多重器官危害 1. 臨床評估標準 臨床上,精密測量內臟脂肪需仰賴電腦斷層掃描(CT)或雙能量X光吸收儀(DXA)。但在日常與基礎醫學評估中,通常使用以下替代指標: 腰圍(Waist Circumference): 亞洲成年男性腰圍 >90cm、成年女性腰圍 >80cm,即提示內臟脂肪過度堆積。
